Please attach a list of all current household income and how often it is received in gross amounts.  Also attach a copy of your most recent 1040 tax return.  If you are having unusual financial difficulties or need special consideration please explain.

 
Income must include wages, salaries, tips, unemployment compensation, workers compensation, net income from self-owned business, pensions, supplement security income, retirement income, veterans payments, social security, disability benefits, income from estates/trust/investments, interest/dividends, welfare, child support, alimony and all other  income sources.
